TeleRare Health, LLC does not provide medical care directly. Clinical services available through the TeleRare Health Platform are provided by licensed clinicians employed or contracted by TeleRare Health PC and by our partner healthcare organizations (such as hospitals and medical groups).

If you are receiving care as a patient of a hospital or health system, that organization’s Notice of Privacy Practices (NPP) applies to your care. Your hospital or health system is required by law to provide you with its NPP and to make it available upon request and typically on its public website.

If you are receiving care directly from TeleRare Health PC (outside of a partner health system), the TeleRare Health PC Notice of Privacy Practices applies to your care. See below for the current version and historical versions.
